    Microelectrode Recording for Deep Brain Stimulation of the Subthalamic Nucleus in Patients with Advanced Parkinson's Disease:Advantage or Loss of Time?

    AIM: To investigate the effect of using microelectrode recording (MER) on the length of time required to carry out a deep brain stimulation (DBS) procedure of the subthalamic nucleus in patients with Parkinson's disease (PD). MATERIAL and METHODS: The time required to include MER in the DBS operation was calculated for the first and second sides in 24 patients with PD. The number of microelectrodes used on each trajectory for the first and second sides, and the percentage of permanent electrodes implanted on each trajectory for the first and second sides, were quantified. RESULTS: The average times taken to use MER were 23.4 +/- 6.2 minutes, 17.4 +/- 6.5 minutes, and 41.2 +/- 6.3 minutes for the first side, second side and total procedure, respectively. In 75% of patients, the permanent electrode was implanted at the planned target site for the first side, and in 61% of patients for the second side. CONCLUSION: MER extends the time required to carry out the DBS procedure. However, during surgery, it provides real-time information on the electrodes' neurophysiological locations and helps the surgical team choose an alternative target if the planned target does not produce satisfying results

    The specification, the deployment, the maintenance and the evolution of technical information systems which support the processes of products developement (PLM) constitute complex tasks for organisations like SME or innovative companies. If today the support on a software package developed and maintained by an editor allows long-lasting solutions, researchers face the issue of the convergent evolutions of the business processes and the software package. As the technical evolutions embedded in digital engineering, this thesis proposes methodologies to be implemented to facilitate this double axis of evolution. Within his innovation framework, the software editor wishes to provide his customers with PLM systems evolution tools.This project, granted as an industrial thesis with AUDROS company, aims at supplying the concepts and the tools which simplify the synchronization of the various business tools within the information system. The company can thus be dynamically supported in a context of extended enterprise.Le déploiement, la maintenance et l'évolution des systèmes d'informations techniques qui accompagnent les processus de création de produits (PLM) constituent des tâches complexes et parfois onéreuses pour des structures de type PME ou micro entreprise innovantes. Si l'appui sur un progiciel développé et maintenu par un éditeur permet aujourd'hui des solutions pérennes, la question de l'évolution conjointe des processus métiers de l'entreprise et du progiciel suite aux évolutions techniques de l'ingénierie numérique pose aux chercheurs la problématique des méthodologies à mettre en œuvre pour faciliter ce double axe d'évolution. Dans le cadre de ses démarches d'innovation, l'éditeur progiciel souhaite rendre accessibles à ses clients PME les capacités fonctionnelles de sa solution PLM en développant un atelier de modélisation pour la création de modèles métier au sein des systèmes PLM et la gestion de leur cohérence au cours du temps.     The clinical, cytogenetics and molecular characterization of inverted duplication/deletion of chromosome 8p in a boy with mental and motor retardation: Genotype-phenotype correlation in a case report

    Background: Rearrangements that occur mainly through the non-allelic homologous recombination (NAHR) during maternal meiosis in short arms of chromosome 8 is relatively associated with various clinical spectrum.Aim: The objective of this study was to report cytogenetics and molecular characterization of a mental and motor retarded boy with short arm of chromosome 8 rearrangements [invdupdel(8p)] in this current case report. Subjects and methods: We report an 11-year-old boy with scoliosis, intellectual disability, mental-motor retardation and characteristic facial features. Agenesis of corpus callosum was detected with brain Magnetic Resonance Imaging (MRI) analysis. Derivative chromosome 8 structure was identified after conventional cytogenetics – karyotype analysis, Multiplex Ligation-Dependent Probe Amplification (MLPA) and Microarray-based Comparative Genomic Hybridization (aCGH) techniques. Genotype-phenotype correlation in the current proband case will be discussed.Results: Case was diagnosed as 46, XY, der (8), del (8) (p23.1) invdup (8) (p11.1-p23.1) by using advanced comparable techniques. Subtelomeric MLPA analysis showed deletion of FBXO25 gene which is located at 8p23.3 locus and FISH with subtelomeric probes for 8p shows also only deleted region. The microarray- CGH profilling showed 7,9 mb deletion for 8p23.1 and 31 mb duplication for 8p11.1 locuses.Conclusion: Results from the current case emphasized that the cases with clinical manifestations of such disorders extremely need to be examined by combined comparable genetics techniques such as; karyotyping, FISH, MLPA and chromosomal microarray for the accurate phenotype – genotype correlation.Keywords: Chromosomal rearrangement, Corpus callosum, Invdupdel(8p)Array-CGH, MLP

    Burnout in Turkish Adult Neurology Specialists

    Aim:Burnout in medical doctors may worse affect patient care or physical performance of clinician. We aimed to investigate the burnout ratio and the factors associated with burnout in Turkish neurology specialists.Materials and Methods:The neurology specialists in Turkey were included in the study. The participants were asked to fill a questionnaire comprised of 33 questions regarding various thoughts and experiences. The participants gave a response to the questions as follows: strongly disagree, slightly agree, moderately agree, strongly agree, completely agree. According to the meaning value of the question (negative or positive meaning), the answer was given a point in a range of 1-5. Sum of points divided by the maximum point (165) gave a burnout ratio.Results:The mean age was 38.78 (±8.42) years, and the female/male ratio was 461/255. The mean burnout ratio was found to be 46.73% (±8.95). Male sex, academic membership, higher academic degree, working in medical faculty hospital, lower salary, being single or nonparent, nightshift, absence of on call work, or working in the intensive care unit were detected to be associated with a higher burnout ratio. Burnout ratio was in positive correlation with age, number of patients examined, and duration of working hours, but in negative correlation with number of auxiliary staff or neurologists in hospital.Conclusion:Our study is the first study to demonstrate a high burnout ratio in a large sample of Turkish adult neurology specialists. Being male, older, academician, professor, single or nonparent, working in medical faculty hospital or in intensive care, low salary, nightshift, and high patient number or working hours seem to be associated with burnout

    Sex difference and intra-operative tidal volume: Insights from the LAS VEGAS study

    BACKGROUND: One key element of lung-protective ventilation is the use of a low tidal volume (VT). A sex difference in use of low tidal volume ventilation (LTVV) has been described in critically ill ICU patients.OBJECTIVES: The aim of this study was to determine whether a sex difference in use of LTVV also exists in operating room patients, and if present what factors drive this difference.DESIGN, PATIENTS AND SETTING: This is a posthoc analysis of LAS VEGAS, a 1-week worldwide observational study in adults requiring intra-operative ventilation during general anaesthesia for surgery in 146 hospitals in 29 countries.MAIN OUTCOME MEASURES: Women and men were compared with respect to use of LTVV, defined as VT of 8 ml kg-1 or less predicted bodyweight (PBW). A VT was deemed 'default' if the set VT was a round number. A mediation analysis assessed which factors may explain the sex difference in use of LTVV during intra-operative ventilation.RESULTS: This analysis includes 9864 patients, of whom 5425 (55%) were women. A default VT was often set, both in women and men; mode VT was 500 ml. Median [IQR] VT was higher in women than in men (8.6 [7.7 to 9.6] vs. 7.6 [6.8 to 8.4] ml kg-1 PBW, P < 0.001). Compared with men, women were twice as likely not to receive LTVV [68.8 vs. 36.0%; relative risk ratio 2.1 (95% CI 1.9 to 2.1), P < 0.001]. In the mediation analysis, patients' height and actual body weight (ABW) explained 81 and 18% of the sex difference in use of LTVV, respectively; it was not explained by the use of a default VT.CONCLUSION: In this worldwide cohort of patients receiving intra-operative ventilation during general anaesthesia for surgery, women received a higher VT than men during intra-operative ventilation. The risk for a female not to receive LTVV during surgery was double that of males. Height and ABW were the two mediators of the sex difference in use of LTVV.TRIAL REGISTRATION: The study was registered at Clinicaltrials.gov, NCT01601223
